HellO! change the {name} to YOURURL.tumblr.com then put it wherever u want. :) in the sidebar;navigations, etc. :)

simple. okay, this is how my code looks like :
"http://www.tumblr.com/ask_form/savemygreen.tumblr.com"
you can adjust the width and height. or you can also adjust this : scrolling="no" to this : scrolling="yes"
If your using this and it doesnt work , your ask isn't probably on. Find it when you customize , somewhere in the community.
